Every year, millions of people get sick and many die from influenza, also known as the flu. One of the best ways to prevent the flu is through vaccinations. Once again, we will work with the health department and the Maryland Partnership for Prevention (MPP) to offer flu vaccinations at your child’s school.

The clinic at your child’s school will be held on Wednesday, October 26, 2022. If you want your child to receive a flu vaccination, fill out an online consent or a paper form. Do NOT fill out both.

1. Read the Vaccine Information Statement, which can be found at https://tinyurl.com/MDFluVIS or in your school nurse’s office.

2. Fill out and return the form (copies are available in the front office). Be sure to:

· Fill out every section. Write neatly and use ink.

· Return the consent form by Tuesday, October 18, 2022
